Trans Abstract

Background

The purpose of this study was to examine undergraduate medical students’ perceptions and intentions regarding patient safety during clinical clerkships.

Methods

Cross-sectional and self-administered questionnaire survey was conducted on 34 students from one medical school using a modified version of the Medical Student Safety Attitudes and Professionalism Survey (MSSAPS). We assessed 4rd-year medical students’ perceptions of the cultures (‘safety’, ‘teamwork’, and ‘error disclosure’), ‘behavioural intentions’ concerning patient safety issues and ‘overall patient safety’. The overall response rate was 66.4%.

Results

Among safety domains, “teamwork culture” was rated highest. “Error disclosure culture” received the lowest ratings. Regarding the error disclosure domain, only 10% of respondents reported that they have received education or training on how to disclose medical error to patients. Independent of survey domains, when students were asked “Overall, do you think your hospital is safe based on your clinical rotation?”, 61.8% reported that the hospital was safe.

Conclusions

Assessing students' perceptions of safety culture can provide clerkship directors and educators with information that enhances the educational environment and promotes patient safety. Discussions of medical errors, patient safety, and how best to incorporate an analysis of these issues into the existing curriculum are needed.

Ⅰ. 서 론

환자가 안전한 환경에서 안전한 의료서비스를 받는 것은 기본적인 권리로 모든 환자와 가족들은 병원에서 의료서비스를 받는 동안 안전하고 완벽한 보호와 돌봄을 기대한다. 그러나 현대 의학이 복잡해지고 불연속성으로 인한 문제가 심각해짐에 따라 환자안전에 대한 우려도 같이 증가하고 있다[1]. 따라서 최근 진료 현장은 물론 의학교육 분야에서도 환자안전 교육과정의 도입과 확대에 대한 논의가 이루어지기 시작하였다[2]. 최근 세계보건기구에서도 의료 오류를 예방하고 환자안전을 향상시키기 위해 의과대학에서 현실적으로 적용가능하고 체계적인 환자안전 교육 프로그램을 개발하고 실행할 것을 권고한 바 있다[3].

환자안전은 첨단장비의 구축과 같은 기술적 해결책뿐만 아니라 의사, 간호사, 병원경영진의 인식 전환을 전제로 하는 환자안전문화의 조성과 이를 위한 교육과 훈련이 동반되어야 한다. 리더십, 조직 내 협력체계, 의료과오에 대한 의사소통의 개방성 등 안전에 대한 조직문화의 개선이 결여된 채 각종 최신 의학기술, 의료장비와 컴퓨터 등 첨단정보기술 체계의 구축만으로는 예방 가능한 의료과오를 감소하는데 한계가 있다[4].

환자안전을 향상시키기 위해서는 의과대학 학생들이 졸업 이전에 환자안전에 대한 기본적인 지식과 태도, 기술을 습득하는 것이 필요하다. 특히 의과대학생들은 임상실습기간 동안 담당교수와 전공의들과의 상호작용을 통한 비언어적인 메시지, 신념이나 태도 등의 잠재적인 교육과정(hidden curriculum)을 통해 환자안전에 대한 태도나 인식에 영향을 받기 때문에 이 시기에 환자안전에 대한 교육은 특히 중요하다[5]. 그러나 우리나라의 많은 의과대학에서는 이에 대한 과정이 마련되어 있지 않고, 환자안전에 대한 교육이 이루어지는 경우에도 기본적인 지식의 전달에 초점이 맞추어져 있다[6].

환자안전 인식 및 태도에 대한 연구가 국내에서도 수행되고 있지만, 대부분 간호사를 비롯한 병원 근무자들을 대상으로 한 연구로, 의과대학 학생들을 대상으로 한 연구는 부족한 실정이다[7-13]. 따라서 이 연구에서는 임상실습 과정을 마친 의과대학 4학년 학생들을 대상으로 환자안전문화에 대한 인식과 태도를 파악하고, 향후 의학교육과 임상실습 교육 과정에서 환자안전문화 형성을 위해 개선이 필요한 중점 영역을 파악하고자 한다.

Ⅱ. 연구방법

조사도구 및 대상자

설문조사는 의과대학 학생의 환자안전 태도를 조사하기 위해 개발된 Medical Student Safety Attitudes and Professionalism Survey (MSSAPS)을 국문으로 번역하여 사용하였다[14]. 설문지는 ‘환자안전 문화’, ‘팀워크문화’, ‘오류공개 문화’, ‘환자안전 행동의도’, 그리고 ‘전반적인 환자안전에 대한 인식’을 물어보는 5개 영역의 26개 항목으로 구성하였다. 10점 만점인 ‘전반적인 환자안전에 대한 인식’을 제외한 모든 문항은 5점 척도로 ‘전혀 그렇지 않다’는 1점, ‘매우 그렇다’는 5점으로 측정하였다. 안전문화 영역 중 “오류에 대해 토론하는 것은 어려웠다’와 같이 부정적인 내용에 대해 동의여부를 물어보는 문항은 ‘전혀 그렇지 않다’는 5점, ‘매우 그렇다’는 1점으로 역코딩한 후 분석하였다.

조사대상은 충청남도에 위치한 한 개 의과대학에서 임상실습 과정을 모두 마친 의학과 4학년 34명이었다(응답률 87.2%). 조사는 2016년 9월 설문지를 배포하고, 회수하는 방식으로 조사를 진행하였다.

분석방법

5점 만점의 리커트 척도 문항의 경우 “그렇다”와 ”매우 그렇다”로 응답한 경우 설문문항의 내용 대해 동의하는 것으로 정의하였다. 10점 척도인 ‘전반적인 환자안전에 대한 인식’ 문항은 7-10점에 응답한 경우 환자안전에 대한 인식이 긍정적인 것으로 정의하였다[14]. 문항별로 평균, 빈도, 백분율을 산출하였고, 영역별로 긍정적 응답률의 평균값을 제시하였다. SAS software (ver. 9.2; SAS Institute, Cary, NC, USA)를 이용하였다.

윤리적 고려사항

Ethics approval and consent to participate: The study was approved by the Institutional Review Board of Yonsei University Graduate School of Public Health (2-1040939-AB-N-01-2015-305). All participants gave written informed consent to participate.

Ⅲ. 연구결과

설문에 응답한 학생 34명 중 남자는 23명(67.7%), 여자는 11명(32.4%)이었다(Table 1).

환자안전 문화 영역 중 ‘수술실에서는 표준 운영절차, 가이드라인, 또는 프로토콜이 준수되었는지’에 대해 ‘그렇다’ 또는 ‘매우 그렇다’고 동의한 학생은 88.2%이었다. 반면 ‘다른 사람의 오류로부터 학습할 수 있는 문화’라고 동의한 학생은 전체의 29.4%이었다(Table 2).

팀워크 문화 영역에서는 64.7%의 학생이 ‘팀워크 문화가 환자가 받는 의료의 질에 영향을 준다’고 응답하였고, 나머지 문항도 50% 이상의 학생들이 긍정적으로 동의하였다. 환자안전 행동의도 영역 중 ‘나는 나의 동료들에게 진료에 영향을 미치는 의료오류에 대하여 환자나 보호자에게 말할 것을 독려할 것이다’에 긍정적으로 응답한 학생은 67.7%이었다.

오류공개 문화영역에서는 ‘환자나 가족에 의료오류를 보고하는 것은 환자안전의 중요한 구성요소이다’ 문항에는 67.7%, ‘의료오류를 기관에 보고하는 것은 환자안전의 중요한 구성요소이다’문항에는 94.1%가 동의하였다. 반면 나머지 문항에서는 긍정적인 응답이 30% 이하였다. ‘오류가 발생한 경우 환자나 보호자에게 공개되었다’고 응답한 경우는 17.7%, ‘의료오류를 공개하기 쉬운 문화이다’라고 동의한 26.5%이하로 동의하는 경우가 많지 않았다. 특히 ‘환자나 보호자에게 오류를 공개하도록 권장을 받은 적이 있는지’에 대한 문항은 2명(5.9%)만이 동의하였다.

‘평소 병원이 얼마나 안전하다고 생각하는지’에 대한 전반적인 인식을 물어본 문항은 평균점수가 10점 만점에 7.6점이었고, 7점 이상으로 응답한 학생은 61.8%이었다. 그림 1은 4개의 각 영역에 따라 문항별 긍정적 동의율(%)의 평균값을 제시한 것으로, 오류공개문화의 긍정적 동의율이 가장 낮았다.

Ⅳ. 고 찰

이 연구는 임상실습을 마친 의과대학 의학과 4학년 학생들을 대상으로 ‘환자안전 문화’, ‘팀워크문화’, ‘오류공개 문화’, ‘환자안전 행동의도’, 그리고 ‘전반적인 환자안전에 대한 인식’을 물어보는 5개 영역에 대한 인식 및 태도를 조사한 연구이다. 5개 영역 중 오류공개 문화에 대한 긍정적 평가가 가장 낮았는데, 특히 ‘오류가 발생한 경우 환자나 보호자에게 공개되었다’와 ‘의료오류를 공개하기 쉬운 문화이다’라고 응답한 경우 긍정적 평가가 낮았다. 그리고 ‘환자나 보호자에게 오류를 공개하도록 권장을 받은 적이 있는지’에 대한 문항은 2명(5.9%)이 동의하였고, ‘다른 사람의 오류로부터 학습하기 좋은 문화이다’ 문항에도 약 1/3만이 동의하였다.

반면 ‘환자나 가족에 의료오류를 보고하는 것은 환자안전의 중요한 구성요소이다’ 문항과 ‘의료오류를 기관에 보고하는 것은 환자안전의 중요한 구성요소이다’문항에는 대부분의 학생들이 동의하였다. 이를 통해 조사에 참여한 학생들 중 많은 학생들이 의료오류를 투명하게 공개하는 것은 환자안전에 중요하다고 인식은 하고 있지만, 임상실습을 통해 경험한 오류공개 문화는 대체적으로 긍정적이지 않았다고 평가했음을 알 수 있다.

오류공개에 대해 교육을 받은 적이 있다고 응답한 학생은 20% 이하로 낮은 수준이었다. 환자안전 교육은 일회성 강의가 아닌, 지속적인 학습과 경험의 축적을 통해 훈련될 수 있고, 의학교육에 전반적으로 통합되어 운영이 될 때 효과적일 수 있다[15]. 그러나 우리나라의 대부분의 의과대학에서도 이에 대한 과정이 마련되어 있지 않거나, 환자안전에 대한 교육이 이루어지는 경우에도 기본적인 지식의 전달에 초점이 맞추어져 있다[6]. 외국의 일부 의과대학에서도 환자안전 교육과정을 도입하고 있으나[16-19] 아직 미흡한 수준이고, 미국의 경우에도 125개 의과대학 중 약 10% 대학에서만 선택 또는 필수로 교육과정을 운영하고 있다[20]. 향후 학생들이 졸업 이전에 환자안전에 대한 전반적인 개념, 기초적인 지식과 기술을 습득할 수 있도록 우리나라에서도 의학교육과정 도입에 대한 적극적인 검토가 필요하다. 미국과 독일의 경우에도 이 연구의 결과와 같이 환자안전에 대해서는 긍정적 인식과 태도를 보였으나, 오류 보고에 대해서는 상대적으로 낮은 자신감을 보였다[21-22]. 반면, 같은 아시아 국가인 홍콩의 경우 응답자중 53% 학생이 자신들의 오류를 숨기지 않을 것이라고 응답하였고, 61%의 학생은 근접오류도 공개해야 한다고 응답하였다[23]. 이러한 오류 보고 및 공개에 대한 적극적인 태도는 국가 차원에서 오류 보고를 적극 권장, 분기별로 공개, 그리고 5년제 의과대학 교육과정 중 3학년부터 5학년에 걸쳐 환자안전 교육의 실시가 긍정적인 영향을 미쳤을 것이다.

의과대학 학생은 정규수업 뿐 아니라 임상실습기간 동안 담당교수 또는 전공의들의 비언어적인 메시지, 신념이나 태도에 많은 영향을 받는다[14,24]. 임상실습을 담당해야하는 의사들이 환자안전과 오류공개에 대해 체계적인 지식이나 태도, 또는 기술을 갖추고 있지 않으면 미래의 의사인 학생들도 이를 습득할 수 없다. 또한 오류 공개는 매우 난이도가 높은 커뮤니케이션 기술을 필요로 하기 때문에 공식적인 교육과정이 필수적이다[24]. 국내 선행연구에서는 우리나라 의사들이 의료오류 공개에 대해 부정적인 인식을 표현하거나 어떻게 보고하는지에 대해 알지 못하는 경우가 많았다[12,24]. 따라서 학생교육과 더불어 의사를 포함한 병원 인력의 교육과 훈련을 통해 환자안전이 중심이 되는 조직문화를 구축하는 것이 동시에 수반되어야 한다.

병동이나 수술실에서 운영절차, 가이드라인 또는 프로토콜이 잘 지켜지는 지에 대한 문항은 2/3 이상의 학생들이 긍정적으로 응답하였다. 이는 우리나라 의료기관인증 평가문항에 관련 문항들이 포함되어 있고, 조사대상인 병원이 의료기관인증을 받은 병원이었기 때문일 것이다.

이 연구는 한 개 의과대학 학생들의 환자안전문화에 대한 인식과 태도를 조사한 연구로, 대체적으로 임상실습에서 경험한 오류공개 문화의 수준이 낮고 오류공개가 중요하다고 인식하고 있음에도 불구하고, 이를 위한 교육을 받은 경험은 부족하다는 결과를 제시하였다. 이는 향후 해당 의과대학 교육과정에 오류 공개 및 커뮤니케이션 방법을 습득할 수 있는 프로그램을 마련하는 것이 시급하고 중요함을 의미한다. 이 연구는 한 개의 의과대학 학생들만을 대상으로 조사했기 때문에 연구결과의 일반화에 제한이 있을 수 있다. 또한, 설문조사에서 솔직하게 응답하기보다 사회적으로 바람직한 평가를 받을 수 있는 답을 말하려는 사회적 바람직성 효과(social desirability bias)를 무시할 수 없다. 그러나 의과대학 학생의 환자안전 문화에 대한 전반적인 수준의 이해는 물론 개선이 필요한 영역을 파악하여, 향후 맞춤형 환자안전 교육프로그램의 개발에 기초자료로 활용할 수 있다는 점에서 의미가 있다.
